Insurance Information
ACCEPTED INSURANCES
Before making an appointment, it is important that you call your insurance plan to ensure that Dr. Mariana Giron and/or Dr. Jonathan Keenan is a covered provider. Many insurance plans require that you select your PCP provider. Once you ensure your plan takes us and you have selected us as your PCP, use the New Patient Appointment request form.
For all of our patients choosing to use their insurance plan, you are required to leave a credit/debit card on file for that appointment.
Not all insurance plans cover all services. In the event your insurance plan determines a service to be “not covered”, you will be responsible for the complete charge. Upon receipt of notification by the insurance company we will transfer the balance to your credit card on file. To see our Financial Policy statement, click here.

Self Pay Rates
We do accept patients who do not have insurance, and would like to pay out of pocket and pay in full at time of service.
For primary care, your initial visit is $150 and all other office visits are $130. Ultrasounds are $150 for most scans; pricing may vary depending on the type of scan and quantity of scans. Besides the cost of a visit, if you need a PAP, it is an additional $100. For prenatal care, please inquire about our maternity plans. For BioTE hormone pellet therapy, for Weight Loss: Semaglutide/Tirzepatide and any other pricing, please call the office.
