Preferred provider lists

Medicare patients

For our Medicare patients, we ask you to familiarize yourself with what will happen at your Medicare: Annual Wellness appointment. You will need to complete the Medicare Wellness Checkup Health Risk Assessment tool prior to your annual wellness appointment. Click on the links above to educate yourself and to print and complete this assessment at home.

Family waiting room

When visiting the clinic, you and your children can wait in our family waiting area. Your children can play in our child safe area while you wait to be seen. No children under 12 can be left in the family waiting area during your actual medical appointment.

Pregnant mommy/daddy resources

One of our passions is to support all of our pregnant mommies/daddies with as many educational opportunities and material resources as possible. Make sure to ask our team members at check out for educational opportunity resources and for baby wipes and diapers. If you need additional material goods, please let them know.
